Barrie Primary Care

s chcstethThe Barrie Primary Care Team of physicians, nurse practitioners and nurses provide primary health care for individuals and families across the lifespan including young families, individuals living with chronic disease, as well as those living with complex medical and chronic mental health issues. Working with other team members at the Centre, they look at factors in people’s lives that may impact and be barriers to health and wellbeing. 

The team does not offer walk-in services. An appointment is required.

Childrens Clinic

The early years of a child’s life are full of growth and change. Our Children’s Clinic is dedicated to supporting children from birth to age five and their families during this important time.

We provide a welcoming, family-friendly space where parents and caregivers can ask questions, get support, and feel confident about their child’s health and development.

Healthy Aging Team

The Healthy Aging Team provides specialized geriatric primary health care for individuals aged 60+ through in-office appointments as well as outreach for those seniors who are homebound (residence, LTC, retirement home, LOFT). Complex geriatric and cognition consultations are provided for BCHC clients as well as in collaboration with the Aging Well Clinic at the Barrie Community Family Health Team.

Physiotherapy

Physiotherapy programs target those facing barriers to access for individual assessment and treatment for BCHC clients or members of our community that do not have third party insurance, access to MVA or WSIB benefits or cannot access the Community Physiotherapy clinics.

Physiotherapy group programs include the evidenced based GLAD program for individuals with chronic OA of the knee of hip.

Dietitian Counselling

Diabetes website picDietitian services include assessment and counselling for clients of the BCHC as well as the community (if capacity permits) that do not have access to a Registered Dietitian. Group programs are also offered. For more details please see connectbchc.ca.

Social Work

(For BCHC primary care clients only)

The Registered Social Workers provide short-term individual, couples and family counselling. The team provides support with managing emotions, chronic mental and physical health issues, trauma, life stressors, work life balance, family issues, and isolation.

Diabetes Education

CapLiving with diabetes—or being at risk—can feel overwhelming. At our Community Health Centre, our Diabetes Team is here to support you every step of the way with caring, practical education tailored to your lifestyle.

Our team includes healthcare professionals such as nurses and dietitians who are specially trained in diabetes care. Together, they provide one-on-one and group education to help you better understand diabetes and how to manage it with confidence.

Diabetes Management

The Diabetes Management Centre sees adults living with Type 1, Type 2 or Gestational Diabetes as well as those who have Prediabetes. We are a team of Registered Nurses and Registered Dietitians who work alongside diabetes specialists (Endocrinologists) to help you manage your diabetes.

Endocrinology

blood dropFor some people, managing diabetes may require more specialized care. That’s where our endocrinology services come in.

At the Barrie CHC, we work with endocrinologists—doctors who specialize in hormone-related conditions like diabetes—to provide advanced support when needed. These services are available to residents of Barrie and the surrounding area through self referral or by referral from your family doctor.

Case Coordinators

The Community Health Case Coordinators will assist with

  • Support in navigating various systems – education, legal, housing and employment
  • Referrals to other community organizations and government services
  • Help with completing applications and forms

Volunteer Services Program

The Barrie Community Centre (BCHC) offers a variety of volunteer opportunities. A BCHC volunteer is a person who is unpaid and participates in a service, program, or sits on a committee of the Centre. Volunteers are recognized by BCHC staff for their unique skill set and knowledge and are placed in volunteer positions accordingly. Each volunteer has a current job description which outlines their role, responsibilities and which staff person they report to. BCHC program staff work closely with volunteers to help them develop new skills and improve their health and well being.
