Living with diabetes—or being at risk—can feel overwhelming. At our Community Health Centre, our Diabetes Team is here to support you every step of the way with caring, practical education tailored to your lifestyle.
Our team includes healthcare professionals such as nurses and dietitians who are specially trained in diabetes care. Together, they provide one-on-one and group education to help you better understand diabetes and how to manage it with confidence.
We focus on giving you the tools and knowledge you need to take control of your health.
Education sessions may include:
- Understanding what diabetes is and how it affects your body
- Healthy eating, meal planning, and reading food labels
- Physical activity and how it supports blood sugar control
- Monitoring your blood sugar levels
- Medications and insulin use
- Preventing complications and staying well
